It is important to be as accurate as possible. We use the information provided to make an initial assessment of the client’s needs. We also use this information to determine whether it is currently safe for the client to engage in trauma therapy.
Please complete the referral form with as much detail as possible. Referrals received with partial or missing information will be rejected and returned to the referrer, which will likely cause a delay in accessing our service.
If inaccurate information is provided, or important details omitted which later come to light, this may result in us having to withdraw support. Wherever possible information will not be shared without consent, except for safeguarding purposes.
Below are our criteria for referral. You must be able to answer ‘yes’ to all of the following to be considered for therapy. If referring from an external agency / organisation, ‘they’ refers to your client.
